Social Worker

To oversee and provide management and social work support to AWWA Home in its services to adult clients with disabilities.

Casework management
- In-take screening of clients, including home visits where necessary
- Case assessment, participate in goal-setting for clients, including taking part in transdisciplinary meetings and setting of ICP for clients
- Compile and maintain clients’ case history and update case records at all times
- Exercise good judgement and discretion in dealing with confidential information

Client Management
- Provide counselling and support to clients and caregivers where relevant and necessary.
- Collaborate with the team in AWWA Home (occupational therapist, physio-therapist, centre manager, pyschologists and nursing staff) to innovate and provide holistic person-centred care for clients

Financial Assessment and Reporting
- Prepare reports and ensure documentation is in place according to funding and audit requirements

Community Engagement
- Develop and maintain collaborations with community partners and stakeholders including other SSAs, hospitals, ministries and parents/caregivers.
- Lead and manage social work functions, outreach and community engagement programmes targeted at the community.

Staff Management
- Manage and supervise a team of social worker associates, ensuring their quality service deliverables
- Provide case supervision to social work associates
- Monitor the performance of staff and provide regular feedback and conduct performance evaluation/appraisal
