Welcome to Tzu Chu Foundation (Singapore) family. We are seeking a compassionate and skilled staff nurse to provide comprehensive home nursing and palliative care. In this vital role, you will deliver high-quality patient care in a home setting, empower caregivers, and ensure the comfort and well-being of our patients.

**Nursing Responsibilities**

**Patient Assessment & Care Planning**:

- Conduct thorough assessments of patients' general health, social history (including financial constraints), and home environment to develop comprehensive care plans.
- Identify and manage physical symptoms such as pain, nausea, and shortness of breath to enhance patient comfort.
- Provide clear guidance to clients and caregivers on care plans to ensure continuity.
- Proactively anticipate and resolve potential problems, escalating to the medical team when necessary.

**Clinical Procedures & Management**:

- Perform a wide range of nursing procedures, including wound dressing and management (e.g., NPWT, IDC, NG tube care, PICC, tracheostomy care).
- Plan and execute routine home care visits based on patient acuity and needs, responding promptly to subacute situations.
- Collaborate with Home Care Medical doctors for joint visits as required.
- Accurately document visit observations, procedures, and action plans, including updating electronic records, wound images, and vital signs.
- Manage and monitor the availability and usage of medical supplies and equipment

**Caregiver Empowerment & Education**:

- Develop therapeutic relationships with patients and families.
- Provide customized health education and training to caregivers, enabling them to safely and effectively manage patient care.
- Assess caregiver training needs and implement systematic training programs in collaboration with caregivers and Next of Kin.

**Palliative Care & On-Call Support**:

- Be prepared to respond to patients with palliative care needs after working hours and on weekends.

**Requirements**:

- Registered with the Singapore Nursing Board.
- Excellent communication skills and a strong learning attitude, open to feedback.
- Adept at making sound clinical judgments regarding patient care and caregiver competencies.
- Demonstrates a keen interest in supporting End-of-Life care for homebound patients.
- Possesses compassion, a positive, and caring attitude towards patients, internal, and external clients.
- Ability to provide comprehensive patient and family education on disease prevention, disease processes, and rehabilitation.
