Case Manager

**Key Responsibilities**
- Conduct needs and intake assessment to identify key risks for intervention
- Develop holistic care plans with caregivers and facilitate the planning of long term financial provision for their dependents with special needs
- Facilitate the setting up of Trust for clients, review their care plans and provide case management support to them
- Navigate through various help systems and facilitate the access to resources and services
- Document and maintain case files and prepare reports in accordance to Organisation standards
- Undertake and perform other tasks and duties as assigned based on the Company needs
**Technical Competencies**:
- Good and effective oral presentation and written communication skills
- Basic understanding of Will and CPF would be an advantage
**Core Competencies**:
- A good team player
- Service oriented with good interpersonal skills
**Preferred achievements / characteristics**:
- Recognised Degree in Social Work or Graduate Diploma in Social Work
- Registered as or qualifies for accreditation as Registered Social Worker with Singapore Social Work Accreditation and Advisory Board (SWAAB)
- Interest in disability and elderly sector
