- Engage community partners such as government agencies, grassroots, other social service organisations to promote dementia-friendly initiatives
- Plan and execute outreach initiatives with and within the community to promote awareness of mental health issues, especially dementia
- Conduct mental health screenings to identify those who are at-risk/diagnosed with mental health conditions, especially dementia; and connect them with relevant services for early diagnosis and treatment
- Build rapport and monitor the conditions of clients with dementia and/or their caregivers
- Ensure proper maintenance of case documentation
- Manage volunteers who might help in the outreach events and other activities when necessary
**Requirements**:
- Degree Holder preferably with some social service experience
- Passionate in raising awareness of mental health issues, especially dementia. Previous outreach experience would be desirable
- Competent in public-speaking, writing and communication skills
- Keen in brainstorming and executing outreach and networking initiatives
- Good interpersonal skills
- Dynamic team player who can also work independently
- This position is on a contract basis
