To implement the National PD Home Support Programme and ensure smooth operational and clinical aspect of the programme.
**MAJOR DUTIES AND RESPONSIBILITIES**
**(A) Administrative (70%)**
- Initiates, administer and implement the programme in line with KTPH practices, policies and protocols and National PD Home Support Programme
- Develop patient monitoring tools necessary for patient's treatment (if necessary)
- To be the key liaison personnel working with doctors, nurses, renal coordinators and service providers to ensure smooth operational and clinical aspect of the programme
- To ensure a consistent scope of PD home support (HS) services for PD patient
- To ensure scalability and sustainability of PD HS
- Monitor the outcomes and key performance indicators for the programme and review the key performance indicators for continuous improvement
- Establish a framework for data collection and analysis on the care delivery process
- Review and ensure that the policies and procedures pertaining to the programme are documented, saved, established, strictly adhered to and updated as required
- Provide regular updates to the Programme Director and on the clinical outcome and the statistical reports as required
- Participates in the relevant IT developments for the programme
**(B) CLINICAL (30%)**
- To coordinate peritoneal dialysis (PD) catheter insertion/removal with nephrologist/urologist, anaesthetist nurse and PD nurse to ensure smooth and timely delivery of patient care
- To coordinate with PD nurse and patient on PD training date post Tenckhoff catheter insertion
- Schedule PD patient appointments, follow-ups, surgeries, admissions, and visits for tests and routine PD maintenance tasks such as change of transfer set
- Being the liaison personnel between patient and home visit service provider
**(A) EDUCATION, TRAINING AND EXPERIENCE**
- Must have attained professional qualifications in Administration and/or Management OR Degree in Nursing, Biomedical Science, Life Sciences, Biotechnology or General Science
- Must have at least 5 years prior working experience in a healthcare environment and have assumed some managerial duties before.
- Good PC knowledge is an advantage
- Some renal experience preferred
- Able to resolve problems in all the aspects of the implementation of the programme including all operational issues, IT issues, clinical related issues
- Able to work with other healthcare providers in the implementation of the programme so as to deliver all key performance indicators
