Singapore St. Hilda's Community Services Centre Full time

Carry out the programes of the respective centres
- Facilitate, guide and assist the clients in their activities
- Participate in development and implementation of group activities
- Assist in conducting group exercise / activity programme
- Assist in conducting therapy programme / intervention for clients
- Assist in medical administration and basic hygiene care
- Assist to provide basic counseling to clients & their families
- Assist to provide case management for client to support them
- Involve in developing Individualized Care Plan for client with other team members by assisting in identifying their needs, strength, limitations and potential and implement intervention plans with SMART goals.
- Measure & monitor clients’ vital signs include Blood Pressure (BP), Heart Rate (HR) or Pulse Rate (PR) & Respiratory Rate (PR).
- Assist in identifying patient with early onset of diseases or condition
- Assist to befriend & provide mental or social support to the clients / caregivers
- Assist to conduct in-service training sessions for staff and volunteers

**Job Requirements**:

- Diploma in social service or related discipline, with a social service background
