**Key Position Objectives**

Provide counselling and case management of clients of Club HEAL.

**Duties & Responsibilities**
- To provide assessment, psychosocial interventions which includes case management and appropriate therapy(s) for clients with mental health issues and dementia, including their caregivers.
- Develop individualised intervention programmes (psychotherapy and counselling)
- Case manage and work closely with the multidisciplinary recovery/dementia team for each client and their caregivers (assess needs and develop care plans)
- Provide clients and their caregivers with education, training and support to equip and enable them to live well in the community
- Liaise with other service care providers of PMI/caregivers as needed
- Up-to-date reporting of each PMI/caregiver
- Required to work closely with and utilise existing community mental health system and process for helping to coordinate patients to other forms of care if necessary.
- Any other duties assigned by COMIT Manager

Preferred achievements / characteristics:

- Bachelors or Masters in Counselling, Psychology or Social Work - 2-3 years of relevant working experience in Social Service Sector or Mental Health Sector.

**Any other duties as assigned by Reporting Officer.
