Assistant Manager, Precinct Planning

The Job Holder is responsible for managing the development and implementation of initiatives to enhance the integration of care and to improve population health. This will be achieved through community health plan in respective precincts within the western region of Singapore
Key Responsibilities:
- Develop and implement community health plan for the precincts within Jurong West, with emphasis on preventive health and social health integration.
- Identify partners for the development of healthy precincts in Jurong West and collaborate with them to enable successful on-ground activation.
- Track and review progress on implementation of community health plan.
- Conduct research on precinct characteristics and resident profile to identify the needs and opportunities within the precinct.
Required Competencies:
- Analytical abilities to integrate information from different sources.
- Adept in stakeholder management to foster buy-in and collaboration.
- Strong writing and verbal communication abilities.
- Resourcefulness in problem solving and resilient in overcoming challenges.
- Knowledge and experience in stakeholder and project management.
- Knowledge of Singapore healthcare system and/or population health models will be advantageous.
- At least 3 years of relevant working experience.
