Job Title: Social Worker

We are seeking a skilled Social Worker to join our team in Choa Chu Kang. This role involves working with clients and their families, providing casework management, and coordinating with external agencies.

About the Role

  • This is a full-time position, working 5 days a week, office hours.
  • The ideal candidate will have a degree in Social Work and 1-2 years of experience in the field.
  • A background in residential care for the elderly is an advantage.

Responsibilities

  • Review applications from various sources, conduct interviews, and assess client needs.
  • Build relationships with clients and their families, provide support, and educate them on accessing resources.
  • Coordinate with external agencies, such as hospitals and clinics, to provide financial assistance to clients.
  • Develop individual care plans, provide therapeutic support, and work with multidisciplinary teams.
  • Monitor progress, develop programs, and ensure alignment with company values.
  • Collaborate with PT and OT departments to enhance client comfort and confidence.
  • Document progress, evaluate clients, and update records.
  • Provide person-centered counseling, risk assessment, and financial management services.
  • Perform other tasks as assigned by management.

Requirements

  • Degree in Social Work.
  • 1-2 years of social work experience.
  • Background in residential care for the elderly is an advantage.
