Community Care Associate (Day Care Centre)We are seeking a Community Care Associate to work in our Day Care Centre. The successful candidate will facilitate and support all aspects of clients' activity of daily living (ADLs), taking into consideration individual preferences. Meal and tea break preparation and serving. Assist client in oral feeding. Assist client in personal hygiene activities, such as toileting, diaper changing, emptying urinary catheter / stoma bag, grooming and showering. Delivery of NGT/ PEG feeding. Perform medication assistance. Monitor, record, and report temperature, pulse, and blood pressure, and any abnormalities to staff nurse or therapist. Cognitive and behavioural monitoring and interventions. Simple wound care management under the supervision of staff nurse. Assist nurses or therapists to provide routine care for clients and improve clients’ quality of life. Support planning and implementation of daily programmes at the day care centre. Report and escalate any client or staff-related adverse incidents. Support non-clinical tasks necessary for daily operations. Requirements include: Experience working in a geriatric setup, long-term care, or nursing home is an advantage but not necessary. Able to multitask and work in a stressful environment. Able to maintain the honour, interest, and dignity of the institution or organisation. Able to serve clients in need, regardless of race, nationality, religious, or social status. Compassionate and able to relate well with sick patients. Caring, understanding, patient, empathetic, and a good team player. Cooperate with colleagues towards total health care of the community.
