Care Planning Coordinator

Competitive Remuneration Package Good Career Exposure and Growth Convenient Working Location As a Care Planning Coordinator, you will be responsible for the following duties: Work collaboratively with internal teams to ensure seamless coordination and delivery of services. Handle enquiries regarding Health services and facilitate the arrangement of requested support. Assess client needs using established screening tools and recommend appropriate Health service packages. Coordinate service referrals by liaising with Health divisions and external partners, submitting referral documents, and tracking follow-up actions. Carry out routine home visits and telephone check-ins to maintain engagement and monitor client progress. Partner with social and healthcare agencies to identify and recommend suitable care programmes for clients. Provide guidance, counselling, and education to caregivers on effective care strategies, available resources, and financial assistance schemes. Work closely with the Operations and Nurse Manager to support and coordinate clients' discharge planning. Contribute to programme development and ongoing evaluation efforts to enhance service quality and effectiveness. Lead or support initiatives aimed at boosting revenue, expanding service uptake, and strengthening overall service utilization. Requirements Min. Diploma in healthcare-related field. Prior working experience in community healthcare settings is advantageous.
