DESIGNATION : Care Coordinator (Administration) (Daycare)

RESPONSIBILITIES

You are responsible to provide service coordination management, administrative support, and operations improvement and analysis for Day Rehabilitation Centres (DRC), with a reporting line to Day Care Ops/Allied Health taskforce.

a) Service Coordination Management
- To work closely with internal stakeholders when applicable to ensure the smooth delivery of service, i.e. Active Rehabilitation (AR) service is carried out by Physiotherapist/Physiotherapist Associate.

b) Administrative Support
- To plan for and remind stakeholders on reassessment of care plans with clients as per their service plan’s requirements. This is to determine if the client requires to continue to receive the service.
- To ensure that proper documentation on the systems and paper filing are completed on a timely basis.
- To provide scheduling and planning support for Day Rehabilitation Centres in accordance with planning guidelines.
- To assist the Rehabilitation and Physiotherapy team with client enrolment and center schedule management, and B3 administrative and support tasks.
- To provide any other ad-hoc administrative general support to the Rehabilitation and Physiotherapy team as assigned.

c) Operational Improvement and Analysis
- To extract, measure and monitor key performance indicators for Rehabilitation and Physiotherapy services.
- To assist Rehabilitation and Physiotherapy centers in driving center utilization metrics.
- To identify areas for operational improvement to increase efficiency of Rehabilitation and Physiotherapy centres.

d) Additional Responsibilities Operational Improvement and Analysis
- Prepare ad-hoc reports and other administrative work as assigned by the Senior Day Care and Home Care HQ.
- Assist Care Coordinator team in service coordination management. According to Senior Day Care authority matrix.

QUALIFICATIONS
- Diploma or Advanced Diploma, preferably in healthcare related disciplines.
- Minimum 3 years’ experience working in a healthcare settings in Singapore
- Good knowledge of the geriatrics and community services

OTHER INFORMATION
- Good interpersonal and writing skills
- Good team player and multi-tasker
- Good communication skills (oral and written)
- Knowledge of LEAN/Six Sigma/AGILE/Design Thinking methodology preferred
- Passion in community services and service oriented
- Good judgment and decision making, problem solving skills
- Independent, resourceful and self-motivated
- Meticulous and have an eye for details
