Health Care Advocate

As a key member of our multidisciplinary team, you will provide holistic and patient-centered care, empowering individuals to take charge of their health through innovative technology solutions. Your expertise in social determinants of health will drive positive outcomes by raising awareness and improving accessibility to resources.

Key Responsibilities:

  • Foster strong relationships with internal and external stakeholders, including patients, caregivers, and healthcare professionals.
  • Contribute to achieving clinical outcomes for preventive health screenings and reducing readmissions and escalations to tertiary care.
  • Collect and analyze social determinants of health data to inform decision-making and improve health outcomes.
  • Utilize technology to enhance health literacy and access to resources, promoting informed decision-making and self-management.
  • Assist in coordinating patient care activities, including tracking and documenting clinical indicators and care provided.
  • Perform basic parameter measurements and close preventive screening and vaccination gaps in relation to national programs.
  • Provide basic health education to improve patients' knowledge on lifestyle management, social and physical activities through social prescribing and health monitoring devices.

Requirements:

  • Diploma or equivalent qualification.
  • Qualification in health promotion is advantageous.
  • Previous experience in healthcare is preferred.
  • Excellent verbal and written communication skills.
  • Pleasant disposition and personality.
  • Strong interpersonal skills, positive attitude, and teamwork abilities.
  • Good communication and coordination skills.
  • Self-motivation and interest in patient care.
  • Proficiency in English and local language/dialect.
  • Microsoft Office proficiency, including Excel, Words, and Power Point.
  • Aptitude in navigating IT programs.
