Population Health Management Professional

We are seeking a skilled and experienced professional to join our team as a Population Health Management (PHM) expert. The successful candidate will be responsible for driving population profiling to inform system planning and care delivery, ensuring the effective use of population health data to draw insights for strategic planning.

The PHM expert will assist in the development and maintenance of PHM profiling products, consolidating and reporting population health indicators performance. They will also facilitate strategic planning and provide support for Population Health Data and Analytics User Group and Health.

In this role, you will have the opportunity to work on exciting projects that impact the lives of individuals and communities. You will be part of a dynamic team that is passionate about making a difference in healthcare.

Requirements

  • Bachelor's degree in Mathematics, Public Health, or Statistics preferred
  • Experience in population health or healthcare planning roles an asset
  • Strong analytical and communication skills
  • Ability to manage multiple projects with effective time management

Preferred Skills

  • Working knowledge of Microsoft Office
  • Experience in data analytics and visualization tools
