This includes ensuring that patients receive comprehensive, individualized care and coordinated access to services. The CMO will collaborate with a multidisciplinary team of doctors, psychologists, nurses, medical social workers, and other allied health professionals to provide timely and appropriate treatment. This role is a full-time service position at Women’s Emotional Health Service+ (WEHS+) / Promoting Parental Emotional Health to Enhance Child Learning (ProPEL) at NUH, with some assessments or interventions possibly conducted in patients' homes. Duties and Responsibilities Provide clinical and psychosocial assessments for patients referred to WEHS+/ProPEL, including diagnosis and clinical goal planning. Screen and monitor patients’ mental status. Provide counseling, psycho-education, and advocacy for patients and families. Assess symptoms, utilize scales, monitor medication adherence, and offer relapse prevention advice. Conduct crisis assessments and interventions. Develop care plans in collaboration with clinicians and specialists, following clinical guidelines for each sub-specialty. Ensure timely diagnostics, treatments, and consultations. Coordinate multiple discipline services and resources (e.g., FSCs, CRSS, Feiyue, Mother and Child Centre). Plan and coordinate home visits with patients and families, executing visits as scheduled. Deliver home interventions and psycho-education based on identified needs. Update the team during multidisciplinary meetings with findings from home visits and follow-up actions. Support patients and families through ongoing home visits and phone calls until discharge is approved by the team. Collaborate with physicians and nurses to ensure stable patients are appropriately transitioned to community care. Handle patient inquiries regarding treatment and referrals. Organize and promote patient education, ensuring materials are current. Maintain an updated list of community resources relevant to patients' areas. Serve as the liaison point between patients, families, and hospital departments, including O&G, MSW, and community partners. Initiate and coordinate discharge planning. Manage and maintain patient data for clinical and research purposes. Conduct research, literature reviews, prepare interview questions, collect data, and compile reports. Assist with DSRB submissions. Track, report, and analyze clinical quality indicators, providing regular statistical updates. Participate in projects related to case management and clinical pathways. Perform additional duties as required for clinic operations. Follow best-practice guidelines and evidence-based practices. Job Specifications/Requirements Degree in Psychology. Preferably with experience or volunteering in the social sector, demonstrating good interpersonal skills. Experience in mental health settings (inpatient or outpatient) is advantageous. #J-18808-Ljbffr
