Job Scope / Key Responsibilities The Case Planning & Evaluation Officer (CPEO) plays a key role in supporting the implementation of the Enabling Skills for Life Programme (ESLP) by leading the development, coordination, and evaluation of Individual Development Plans (IDPs) for persons with disabilities (PWDs). The role ensures a person‐centred approach that actively involves clients, caregivers, staff, and allied health professionals (AHPs), while also building staff capability in assessment and planning practices. Key Responsibilities Case Planning & Coordination Develop, coordinate, and implement Individual Development Plans (IDPs) for PWD clients and caregiver engagement plans, ensuring plans are person‐centred and holistic. Design and coordinate engagement plans to involve and support caregivers in the IDP process. Facilitate case conferences and planning discussions with Training Officers (TOs), Training Aides, AHPs, and other stakeholders to align goals and interventions. Documentation & Evaluation Draft, review, and finalise IDPs and caregiver plans in collaboration with staff, clients, and caregivers. Track, monitor, and evaluate client progress against agreed goals, ensuring plans remain relevant and responsive to client needs. Document case records accurately and maintain compliance with organisational and regulatory standards. Analyse outcomes and provide reports on programme effectiveness, identifying gaps and recommending improvements. Capacity Building & Staff Support Collaborate with the Master Trainer to design, coordinate, and deliver training to staff on assessment tools, IDP planning, and case management practices. Provide coaching and guidance to TOs and Aides on implementing IDPs and related processes where needed. Share best practices to strengthen organisational capability in person‐centred planning and evaluation. Collaboration & Service Integration Serve as a key liaison between clients, caregivers, staff, and external professionals to ensure integrated service delivery. Contribute to the refinement of ESLP frameworks, tools, and processes based on on‐the‐ground insights and evaluation findings. Job Holder's Requirement A recognised degree , preferably in Social Work , Counselling , Psychology , or related fields. Training and/or experience in Special Education or working with PWDs is highly valued. Strong skills in case planning, documentation, and evaluation. Excellent facilitation, communication, and interpersonal skills, with the ability to work effectively with diverse stakeholders. Demonstrated ability to lead discussions, provide guidance, and build staff capacity. Organised, analytical, and outcome‐focused, with a strong commitment to person‐centred practice. #J-18808-Ljbffr
