Senior Community Outreach Worker Job Description

We are seeking a skilled and compassionate Senior Community Outreach Worker to join our team. As a key member of our outreach program, you will be responsible for conducting active ageing programs within and outside the centre. Your primary goal will be to provide befriending and outreach services to seniors, ensuring their well-being and connecting them with necessary resources.

You will work closely with our Managers to implement services that meet the centre's performance indicators. Your role will involve:

  • Assisting in the planning, coordination, and evaluation of active ageing programs.
  • Providing one-on-one befriending and buddying services to frail and vulnerable elderly at their homes.
  • Conducting community outreach and need assessments to identify areas of support for elderly individuals within designated service boundaries.
  • Referring elderly individuals to relevant help services when needed.
  • Performing administrative duties such as maintaining attendance records, visitation notes, and data collection as required by authorities.
Required Skills and Qualifications

To succeed in this role, you will possess a minimum Diploma qualification and experience in eldercare, healthcare, or social services. Strong communication skills, both written and verbal, are essential. Additionally, proficiency in dialects (e.g., Hokkien) and basic Malay language is desirable.
