About the Role
We are seeking a skilled home care professional to join our team in providing palliative care services to patients in their homes.
The successful candidate will be part of a dynamic team that works collaboratively to provide comprehensive care to clients, including hands-on bedside care, respite assignments, and assisting the home care team with other nursing cases.
Responsibilities:
- Provide high-quality, patient-centered care to clients in their homes, including administering medication, wound care, and other medical interventions as needed.
- Work collaboratively with the multidisciplinary team to develop and implement individualized care plans that meet the unique needs of each client.
- Assist with respite assignments and provide support to family members and caregivers as needed.
- Conduct hospital or home visits to assess new clients for home care services and provide ongoing care and support.
Requirements
To be considered for this role, you must have:
- A minimum Diploma or Bachelor's Degree in Nursing from an accredited institution.
- A valid registration with the Singapore Nursing Board and a clean disciplinary record.
- Proficiency in Microsoft Office and comfortable with IT systems used in healthcare settings.
