Senior Care Specialist

Serving seniors with dignity and respect, we are seeking an experienced Senior Care Specialist to join our care team. As a key member of our integrated care team, you will be responsible for delivering person-centered care to our seniors, promoting their independence and maintaining their cognitive and physical functions.

Reporting to the Community Care Manager, your job scope includes:

  • Supervising and supporting direct care staff in the delivery of Assisted Daily Living (ADL) tasks;
  • Coordinating and implementing individualized care plans for our seniors;
  • Maintaining a safe and clean environment for our residents;
  • Engaging regularly with seniors to monitor their cognition and functional well-being;
  • Collaborating with healthcare professionals to track and report any concerns or issues;
  • Implementing programs as per guidelines and continually seeking feedback to improve their effectiveness;
  • Tailoring centre-based programs, including intergenerational and volunteer programs, while adhering to approved guidelines;
  • Supporting the preparation of relevant documentation to assist in incident reporting.

In addition to your technical skills and experience, you will possess strong interpersonal and communication skills, both verbal and written. You will be able to work independently and as part of a team, demonstrating a growth mindset and embracing continuous learning and development.

To succeed in this role, you will need:

  • A Diploma in Social Service or related field;
  • Experience managing and leading teams will be an added advantage.
