Job Title: Senior/Assistant Senior Social Worker
We are seeking a highly skilled and experienced professional to join our community mental health services as a Senior/Assistant Senior Social Worker.
About the Role:
- This is an exciting opportunity for a seasoned social worker to take on a key role in managing and coordinating post-discharge programs in partnership with tertiary healthcare institutions.
- You will conduct thorough assessments using the Bio-Psycho-Social-Spiritual (BPSS) model to develop and implement individualized care plans and interventions.
- Deliver high-quality psychotherapy sessions and conduct mental health talks for clients and caregivers, promoting holistic person-centered care.
- You will build and maintain strong networks with stakeholders and community partners to address service gaps and improve care accessibility.
- Supervise the team through regular individual and group supervision sessions.
- Promote ongoing training and professional development of team members aligned with AIC Mental Health Competency Framework.
- Plan, facilitate, and evaluate group work sessions for clients and caregivers.
Requirements:
- Minimum 3 to 5 years of experience in mental health and dementia care with demonstrated competencies in case management, group work, program development, and community engagement.
- Prior supervisory experience is advantageous.
