Senior Case Management Professional

- Location: Mattar Rd (McPherson)
- Working hours: Mon - Thurs 8.30AM-6.30PM, Fri 8.30AM - 5.30PM
This role is responsible for overseeing case management processes, ensuring timely submission, accuracy, and integrity of casefiles.
The successful candidate will conduct case supervision, mentor junior caseworkers, and provide guidance to interns (art therapy, counselling, social work).
The postholder will chair monthly case conferences and support ongoing case discussions.
Liaising with stakeholders, the ideal candidate will ensure effective collaboration and activation of services when required.
The role involves managing ECRM matters, including admissions, discharges, extensions of stay, case assignments, and follow-ups.
Additionally, the successful candidate will oversee CANS administration, including user accounts, assessments, and issue resolution.
The postholder will review and vet reports (e.g., RC4, incident reports) and submit funding applications for higher-risk residents.
Casework data, statistics, and reports on a monthly and quarterly basis will be collated and submitted by the successful candidate.
Promoting proper documentation, records, and reporting systems will be an essential aspect of this role.
Requirements- Preferably someone with 5 years relevant experiences.
- Diploma/Degree in psychology will be a plus point.
An annual performance bonus + year-end bonus + WFH hybrid arrangement are available to the successful candidate.
