Job Opportunity: Senior Case Manager

Job Overview:

The ideal candidate will be a registered nurse with extensive clinical experience and a proven track record of success in healthcare management, who can assume leadership duties.

As the Programme Manager for Kidney Care Services, you will be responsible for all operational aspects of the project. This includes developing and driving the implementation of evidence-based care pathways for advanced chronic kidney disease (CKD) patients.

  1. Develop and implement hospital policies and procedures.
  2. Establish, review and update team standard operating procedures (SOPs), protocols, and case management tools.
  3. Mentor and precept staff to improve their performance and professional development.
  4. Monitor and evaluate staff performance to ensure high-quality patient care.

Key Responsibilities:

  • Programme Management: Develop and drive the implementation of Kidney Care Services.
  • Clinical Skills: Apply clinical skills and critical thinking to deliver high-quality patient care.
  • Team Leadership: Lead a team of Coordinators in implementing programme goals and objectives.
  • Quality Improvement: Participate in quality improvement activities to enhance patient outcomes.

Requirements:

  1. Degree in Nursing or related field.
  2. Registered Nurse with at least 3 years of clinical experience.
  3. At least 5 years of prior working experience in a healthcare environment.
  4. Possess excellent interpersonal and communication skills.
  5. Computer literacy skills – MS Office (Word / Excel / PowerPoint).
  6. Effectively bilingual.
