Job Overview

We are seeking a compassionate and proactive Program Coordinator to join our team. In this role, you will play a vital part in empowering seniors to age in their community.

Key Responsibilities:
  • Community Outreach: Establish relationships with seniors and community members to raise awareness about our centre programmes and services.
  • Social Assessment: Conduct basic assessments to identify socially isolated seniors and those with needs.
  • Community Building: Collaborate with grassroots, community partners, and service providers to build up a community network.
Empowering Seniors

In this role, you will have the opportunity to make a meaningful impact on the lives of seniors. You will organize activities at Eldercare centre and off-site locations to encourage active living.

  • Activity Organization: Organize activities that promote physical and mental well-being among seniors.
  • Volunteer Engagement: Co-create activities with volunteers to engage seniors in various hobbies and interests.
  • Senior Empowerment: Encourage seniors to take an active role in their community by participating in volunteer opportunities.
Befriending and Buddying

You will manage volunteers to provide befriending/buddying services to prevent social isolation among seniors.

  • Volunteer Management: Manage volunteers to provide regular check-ins with seniors, promoting social connections and reducing feelings of loneliness.
  • Relationship Building: Establish meaningful relationships with seniors through befriending and buddying initiatives.
Care and Support

In this role, you will provide essential care and support services to seniors in need.

  • Information Referral: Provide information and referral services to ensure seniors with needs are supported.
  • Case Management: Participate in discussions with case management teams to ensure holistic support for clients.
Administrative Duties

As a Program Coordinator, you will also be responsible for supporting volunteer management processes, including tracking volunteering hours and maintaining databases.

  • Administrative Support: Assist with administrative duties such as data collection and reporting for evaluation purposes.
  • Database Management: Maintain accurate and up-to-date records of volunteer hours and other relevant data.
