Social Care Specialist

We are seeking a skilled Medical Social Worker to join our team. The ideal candidate will have experience in providing bio-psychosocial assessments and interventions, as well as supervising and appraising medical social workers.
The successful candidate will be responsible for undertaking and leading projects, delivering patient care during home visits, and collaborating with healthcare professionals to provide high-quality patient care.
Key Responsibilities:
- Conduct comprehensive bio-psychosocial assessments of patients to identify their strengths and challenges.
- Develop and implement individualized intervention plans to address the needs of patients and families.
- Supervise and appraise medical social workers, executives, and support staff on standards of practice delivery.
- Lead projects to improve patient outcomes and develop policies and procedures to ensure effective service delivery.
Required Qualifications:
- Degree in Social Work.
- Minimum 2 years of experience as a Medical Social Worker.
- Able to start work immediately.
