Social Care Specialist

We are seeking a skilled Social Care Specialist to join our team. As a Social Care Specialist, you will be responsible for conducting thorough assessments of clients and providing counseling and support to help them navigate their social and emotional needs.
Key Responsibilities- Assessment and Support: Conduct home visits or social visits to assess clients' social and emotional needs, evaluate their financial condition, and recommend appropriate solutions to address their challenges.
- Care Coordination: Coordinate with community organizations, government agencies, and individuals to advocate for clients' benefits and welfare, acting as a liaison between patients and support services.
- Education and Empowerment: Educate clients and families about illnesses, treatment plans, and available resources to empower them in managing their health effectively.
- Comprehensive Care Planning: Assess each patient's unique condition, combining medical diagnoses and personal interviews to develop comprehensive care plans tailored to their health status, personal strengths and weaknesses, and emotional state.
- Resource Evaluation and Follow-Up: Research local support groups and resources, matching them to individual patients' needs and preferences, evaluating the effectiveness of services and resources provided, and adjusting plans accordingly.
- A recognized degree in Social Work is essential for this role.
