Role Summary

We are seeking a dedicated and compassionate Social Care Specialist to join our team. As a key member of our multidisciplinary team, you will provide high-quality care and support to individuals with dementia and their carers.

Key Responsibilities:

  • Conduct comprehensive case assessments and develop individualized care plans to meet the unique needs of clients with dementia.
  • Provide psychoeducation, supportive counseling, and service referrals to promote the well-being of clients and their carers.
  • Liaise and collaborate with healthcare professionals, external service providers, and other stakeholders to ensure holistic care management.
  • Document client interactions, progress, and outcomes in accordance with organizational policies and procedures.
  • Participate in team meetings, case conferences, and program evaluation activities to contribute to the development and improvement of services.
  • Assist in the preparation of data and statistics for program evaluation and reporting purposes.
  • Plan and facilitate caregiver support group sessions to promote social connection and empowerment among carers.
  • Support outreach events and talks to raise awareness about dementia care and services.

Requirements:

  • Preferably hold a degree in an allied health profession, social work, or psychology. Senior positions may be considered for this role.
  • Demonstrate excellent oral, written, and interpersonal communication skills.
  • Able to facilitate group sessions and possess strong listening skills.
  • Available to work occasional Saturdays and weekday evenings as required.
  • Passionate about dementia care and working with people.
  • Prior experience in dementia care is highly valued but not essential.
