Job Overview

This role involves providing high-quality patient care in a home-based setting. As a Home Care Staff Nurse, you will be responsible for managing the care plans of patients and making home visits to provide palliative care and recommend psycho-social support to terminally ill patients and their families.

The successful candidate will be part of a dedicated team comprising doctors, nurses, and medical social workers who work together to provide comprehensive professional care to patients at home seven days a week.

Key Responsibilities:
1. Manage the care plan for patients and make home visits to provide palliative care and recommend psycho-social support.
2. Work collaboratively with healthcare professionals to ensure seamless care delivery.
3. Identify and address the needs of patients and their families.

Required Skills and Qualifications:
  • Strong understanding of palliative care principles and practices.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Basic knowledge of case management.
