Job Title: Community Social Worker

Job Description:

  • The primary objective of this role is to work closely with the community and provide outreach services that promote social welfare.

Key Responsibilities:

  • Develop and implement outreach plans to publicise our programmes and establish good relationships and contacts with stakeholders or community partners.
  • Manage referrals, engage referral agencies/persons for intake and assessment, and conduct thorough case assessments using information gathered during intake and subsequent engagement with clients, families, and stakeholders.
  • Collaborate with clients to develop intervention plans, provide counselling and support to address their risks and needs issues, and advocate for their rights and access to essential services within the community and relevant systems.
  • Document all case-related records accurately and maintain confidentiality.
  • Participate in research and evaluation activities by keeping abreast of current trends and participating in research projects.
  • Engage in professional development and education through regular attendance at internal case reviews, case conceptualisation meetings, and clinical supervision. Additionally, supervise interns and/or students on placement.
  • Develop and implement new initiatives aligned with the objectives of the Division.

Requirements:

  • Degree in Social Work or equivalent with a minimum of 2 years' experience in the social service sector or similar settings.
  • Proficiency in MS Office programmes and experience with social work interventions; interest in growing clinical skills.
