Senior Community Care Coordinator

We are seeking a highly motivated and dedicated individual to join our team as a Senior Community Care Coordinator. In this role, you will be responsible for coordinating community care services and developing strong partnerships with stakeholders.

The ideal candidate will have a Bachelor's degree in Social Work, Community Work, Gerontology, or a related field, and at least 3 years of experience in eldercare coordination, community care, or a related field. You will also possess good understanding of community care delivery, integrated service models, and inter-agency collaboration, as well as strong organization skills and a proactive, solution-oriented mindset focused on delivering results.

As a Senior Community Care Coordinator, you will work closely with senior care providers, community organizations, and other stakeholders to develop and implement community care programs that meet the needs of seniors. You will also be responsible for monitoring and evaluating program effectiveness, identifying areas for improvement, and making recommendations for change.
