Lead Care Coordinator Role

As a Lead Care Coordinator at econ-healthcare-asia-pte-ltd, you will play a vital role in providing patient-centric care to home care clients. Your responsibilities will include conducting initial and 6 monthly clinical assessments, formulating individualised care plans, and ensuring accurate nursing documentation.

Clinical Support
  • Conduct initial clinical assessment of new clients and 6 monthly review of existing clients, focusing on comprehensive health evaluations.
  • Formulate individualised and holistic care plan, incorporating relevant medical history and lifestyle factors, and review whenever required.
  • Ensure proper and accurate nursing documentation, adhering to regulatory standards and company policies.
  • Perform nursing procedures in accordance with core competencies as a Registered Nurse, upholding high professional standards.
Administrative Support
  • Assist Homecare Head/Nurse Manager in managing daily operations of the home care department, leveraging expertise to drive productivity and efficiency.
  • Handle home care enquiries, provide administrative support, manage feedback and complaints, fostering exceptional customer satisfaction.
  • Coordinate clients' care arrangements, including rostering and on-call duties, guaranteeing seamless service delivery.
Staff Development and Support
  • Provide training and supervision to care staff, when required, promoting growth and skill development.
  • Plan and coordinate assignments of care staff according to job scope and competencies, optimising team performance.
Quality Assurance and Incident Management
  • Participate in quality improvement projects and initiatives, identifying areas for enhancement and implementing solutions.
  • Identify risks, mitigate and perform service recovery, ensuring client safety and satisfaction.

In this role, you will also be responsible for deputising the Care Manager and performing adhoc duties as assigned by Homecare Head.

To succeed in this role, you will need:

  • Possess a Bachelor Degree in related field and/or Degree or Advanced Diploma or Diploma in Nursing.
  • State Registered Nurse with valid practicing license issued by the Singapore Nursing Board.
  • Valid certification in Basic Cardiac Life Support (BCLS).
  • At least 8 years' experience in management of department and have provided supervision to a team of subordinates.
