Eldercare Programmes Specialist

Join HYPERSCAL SOLUTIONS PTE. LTD. as an Eldercare Programmes Specialist and contribute to the company's mission of providing compassionate and individualized care to seniors.

As an Eldercare Programmes Specialist, you will assist the Centre Manager in driving the eldercare programmes and community outreach. Your key responsibilities will include:

  1. Centre Engagement
  • Align with the organisation and department vision and care philosophy
  • Perform community screening, befriend seniors, and support in outreach activities or public education
  • Conduct health assessments for seniors/clients
  • Establish and maintain good interpersonal relationships with clients, caregivers, volunteers, and the community
  • Ensure the safety and well-being of clients, implement, and monitor progress of clients' individual care plan
  1. Volunteer Management
  • Assist in volunteer management and conduct orientation programme
  • Train volunteers in conducting active ageing activities and befriending for seniors/clients
  1. Programme Planning and Execution
  • Plan, organise, implement, monitor, and evaluate the Centre programmes, activities, and events and work out a regular activity calendar
  • Assist in sourcing and creation of activities to improve and refresh resources or activities
  • Assist and facilitate activities for clients together with the care team
  • Facilitate or conduct classes for senior lifelong learning
  1. Staff Relations
  • Work closely with the care team to display team spirit and ensure cohesive work relationships for the best interest of our clients
  1. Other Duties
  • Participate and receive relevant training to excel in the job functions
  • Any ad-hoc duties assigned by Supervisor

Requirements:

  • ITE/Diploma in relevant disciplines such as Community Care and Social Service, Gerontology, or other relevant fields in Social Sciences
  • 2-3 years of relevant working experience, preferably in an eldercare programme and community outreach or have volunteered previously with seniors in a community setting
  • Excellent interpersonal and communication skills, especially with seniors
  • Proficient in IT
  • Able to converse with elderly in English, Mandarin, and/or dialects (in order to handle queries and concerns from English, Mandarin, and/or dialects-speaking patients and/or clients)
  • Resourceful, proactive, responsible, and client-centric
  • Ability to multi-task and meet deadlines in a fast-paced environment
  • Team player with strong planning and organising skills
