As a key member of our team, you will play a pivotal role in coordinating care services for individuals and their families. Your primary responsibility will be to ensure seamless case management, working closely with healthcare professionals, caregivers, and clients to deliver exceptional service.

">Key Responsibilities
  1. Manage daily calls and enquiries, as well as contact potential clients via phone to understand their needs and share information about our services.
  2. Facilitate comprehensive assessments, evaluations, planning, and implementation of personalized care plans to meet the unique needs of each client.
  3. Develop and maintain strong relationships with clients, their families, and healthcare teams to ensure high-quality service delivery.
  4. Coordinate home visits and schedule sessions for both caregivers and clients, promoting effective communication and collaboration.
  5. Ensure timely and accurate documentation of assessments, plans, and outcomes for the team and relevant systems.
  6. Monitor progress and follow up with caregivers to guarantee prompt reporting to clients.
  7. Provide guidance and support to caregivers to facilitate smooth and effective communication with clients.
  8. Conduct regular follow-ups with caregivers and clients' families to evaluate progress and support continuity of care.
  9. Handle clients' service agreements and payments-related invoices with the finance team.
  10. Participate in company events, educational talks, and training workshops to enhance professional development.
  11. Prepare and submit reports promptly.
  12. Perform any other duties as may be assigned from time to time.
  13. Adhere to company standards, policies, and procedures to maintain effective service delivery.

Requirements
  1. Singaporean or Singapore Permanent Resident only.
  2. Diploma in a related field with relevant work experience.
  3. Proficient in Microsoft Word and Excel.
  4. Strong verbal and written communication skills.
  5. Excellent interpersonal and written communication skills.
  6. Self-motivated, with excellent social skills and the ability to network with various professionals.
  7. Proficient in English and preferably in other local languages/dialects.
  8. Problem-solving abilities.
  9. A team player with a passion for patient care, a willingness to learn, and a positive attitude.
  10. Minimum of 2 years of experience in healthcare, social services, or the hospitality service sector.
Benefits
  • Working hours: 9 am to 6 pm, 5.5 days work, flexi work on Saturday.
  • S$2,500 - S$4,000 (negotiable based on working experience).
