Job Description

We are seeking a skilled professional to engage with key stakeholders in the Intermediate and Long Term Care (ILTC) sector, with the goal of enhancing care integration and delivery in the community.

Responsibilities:
  • Regular engagement with ILTC providers (e.g., nursing homes, day care centers, home care services)
  • Management of collaborations between NUHS and ILTC providers
  • Enhance care integration and delivery in the community
