Community Support Specialist

The Community Support Specialist plays a vital role in supporting seniors living in the community to lead active lifestyles and promote their well-being.

Key Responsibilities:

  • Conduct outreach to seniors in identified residential areas to promote activities that support active lifestyles and promote their well-being.
  • Support activities and outings organised for seniors, including data collection to meet reporting requirements.
  • Develop and implement outreach plans, monthly activities calendars, and annual health and social activity plans for seniors.
  • Review and carry out regular home visits for socially isolated seniors, monitor their health and well-being, and update case status.

Befriending and Buddying

  • Manage referral systems and serve as a point of contact for seniors, answering ground queries and providing effective service referrals.

Volunteer Management and Community Partnerships

  • Drive outreach efforts by recruiting, orientating, training, retaining, deploying, engaging, and appreciating volunteers.
  • Represent the organization in community partnership networking sessions, such as Communities of Care, multi-agency case discussions, and meetings.

Day-to-Day Centre Operations

  • Handle walk-in enquiries and phone calls, maintaining proper records and documentation.

Requirements:

  • Minimum Diploma in any discipline from an accredited institution, preferably in Gerontology, Social / Healthcare Services, Community Development, Psychology, or Social Work.
  • At least 1 year of experience in front-facing, direct operational work in eldercare or social services, with prior volunteering experiences preferred.
  • Proficient in IT and Microsoft Office applications.
  • Good interpersonal skills: resourceful, proactive, responsible, and client-centric, with passion for serving seniors and volunteers.
  • Ability to multitask, work independently, and meet deadlines in a fast-paced environment, with good planning and organisational skills.
