Job Description

The Community Resource, Engagement and Support Team serves as a first mental health touchpoint in the community.

We conduct outreach events and activities to promote mental health awareness, identify, and support those with mental health needs and their caregivers.

As a Social Worker, you are responsible for organizing and conducting outreach activities to increase mental health and dementia awareness, promote early identification, conduct assessments, and provide casement management support for client care.

Key responsibilities include:

  • Coordinating care and support to clients with or who are at risk of mental health issues and/or dementia with the appropriate resources.
  • Assessing and providing intervention to clients, including referring cases to other agencies as needed.
  • Providing emotional support to clients and caregivers and maintaining case documentation.
  • Outreach and networking to the community and stakeholders to promote mental wellness and mental health awareness.
  • Planning and conducting cognitive stimulation activities, mental health/wellness programs, and dementia awareness.
  • Organizing caregiver group sessions and managing administration and finances.
  • Maintaining accurate client records and program data in line with our guidelines, adhering to security guidelines.
Requirements
  • Possessing at least a Degree in Social Work or Graduate Diploma in Social Work.
  • Having at least 2-3 years of experience.
  • Being conversant in English and a second language, with the ability to speak dialect being an added advantage.
  • Exhibiting good communication and interpersonal skills.
  • Being a good team player, outgoing personality, dynamic, and self-motivated with a strong passion for mental health needs.
  • Being eligible for Singaporeans.
