
Community Mental Health Specialist

The role of a mental health professional involves providing individualised care plans, case management, counselling services and support to individuals with mental health conditions, dementia, and their caregivers.
Key Responsibilities:
- Conduct assessments using the Bio-Psycho-Social-Spiritual (BPSS) model to develop individualised care plans and interventions for clients with complex health and social needs.
- Provide case management and counselling services to support clients' recovery journeys and improve their mental wellbeing.
- Deliver talks and presentations to raise awareness of mental health and dementia among communities.
- Support volunteers who befriend clients with mental health needs and provide guidance on how to engage effectively with clients.
- Collaborate with community partners and stakeholders to develop and implement community outreach programs and group work initiatives.
- Plan, organise, and implement community-based projects that promote mental health and wellbeing.
- Undertake any duties assigned by the Head of Community Mental Health and the Executive Director.
Required Skills and Qualifications:
- Experience in working with clients with mental health challenges and person-centred care and recovery is highly valued.
- Strong interpersonal and communication skills are essential for effective collaboration and teamwork.
- Ability to work independently and collaboratively within a team to achieve project goals.
- Good administrative and computer skills with proficiency in MS Office are required.
