Job Overview
The role of a Healthcare Specialist is to provide high-quality patient care in their own homes.
We are seeking a skilled nurse who can work independently, possess excellent communication skills and has the ability to conduct thorough evaluations, administer medications and assess and manage wound care.
Key Responsibilities:
- Conduct thorough patient evaluations, including vital signs and medication management.
- Administer medications as prescribed by the patient's physician.
- Assess and manage wound care, ensuring proper healing and prevention of complications.
- Provide education and support to patients and their families on home health care strategies and procedures.
- Collaborate with healthcare professionals to develop comprehensive care plans.
- Maintain accurate records and communicate effectively with patients, families and healthcare teams.
Requirements:
- Minimum diploma in nursing or related field.
- Registration with the relevant nursing board.
- Ability to travel to multiple patient locations per day.
- Desirable experience in geriatric, ICU, emergency unit, medical surgical ward and palliative care settings.
