Job Opportunity
The Home Care Coordinator plays a vital role in ensuring the delivery and smooth functioning of home care services. This position ensures the efficient management of administrative processes, client care coordination, and service quality.

Main Responsibilities
1. Client Admission & Discharge Coordination
* Conduct intake assessments and manage admission and discharge process for home care referrals, ensuring timely documentation.
2. Service Monitoring & Staff Scheduling
* Oversee daily provision of personal care services to clients and coordinate staff scheduling, including rostering and leave management, to ensure adequate coverage and continuity of care.
3. Operational Support & Administrative Management
* Provide day-to-day operational support and maintain comprehensive records for all clients, including filing and updating documentation as needed.
4. Staff Development & Training
* Assist in implementing training programs and competency frameworks for care staff to enhance service quality and staff performance.
5. Client & Caregiver Engagement
* Obtain regular feedback from clients and caregivers to assess satisfaction and identify areas for improvement, addressing concerns and escalating issues appropriately.
6. Custodial Care Support
* Provide hands-on support in custodial care when required, ensuring clients' comfort and safety.
7. Additional Duties
* Perform other duties as assigned by the supervisor to support the overall functioning of the home care services.

Necessary Skills and Qualifications
* Diploma / Degree in any relevant field.
* Excellent communication and interpersonal skills.
* A team player who is organized and meticulous at work.
* Proficient in Microsoft Office.
* Good understanding of working with elderly individuals.
