Job Title: Senior Community Care Coordinator

About the Role:

As a Senior Community Care Coordinator, you will play a key role in providing emotional support, counseling services, and connecting patients with community resources to enhance their overall wellbeing.

Duties and Responsibilities:
  • Conduct comprehensive assessments to identify patient needs, strengths, and challenges in their home and community.
  • Collaborate with patients, families, and healthcare organizations to develop personalized care plans that facilitate smooth transitions from hospital to home.
  • Participate in multidisciplinary team meetings to coordinate patient care and discuss progress.
  • Organize case conferences with relevant agencies to enhance patient care and service coordination.
  • Provide emotional support, counseling, and group sessions to foster resilience and shared learning among patients and caregivers.
  • Assist patients and families in accessing community services and support to meet their needs.
  • Conduct regular home visits and follow-up sessions to monitor progress and address ongoing concerns.
  • Maintain accurate and timely documentation of patient care plans, interventions, and coordination efforts.

Requirements:
  • Degree in Social Work or related field, preferably with 1-2 years' experience in social work.
  • Experience in healthcare or social services is an advantage.
  • Registration with Social Work Accreditation Board is preferred.
  • Strong interest in working with seniors and their caregivers.
  • Able to work independently and as part of a team.
