Job Title: Community Care Leadership Specialist
About the Role:This is a key leadership position responsible for overseeing a team of care coordinators and case managers to provide comprehensive post-discharge care coordination services. The successful candidate will be an experienced leader with a strong background in community healthcare and proven expertise in managing teams to deliver high-quality patient outcomes.
Key Responsibilities:- Lead a team of care coordinators and case managers to ensure seamless patient transitions from hospital to community settings.
- Develop, implement, and evaluate policies and procedures to optimize support for patients and their caregivers.
- Collaborate with internal stakeholders and external partners to advance community care ecosystems and reduce unnecessary hospital readmissions.
- Provide clinical supervision, manage staff performance, and oversee professional development and training needs to ensure clinical competence.
- Participate in quality improvement initiatives and contribute to research projects aimed at enhancing clinical care standards.
- Professional qualification in Nursing, Social Work, or Allied Health or any relevant clinical discipline.
- No less than 8 years of experience as a care coordinator, case manager, or community nurse in community healthcare settings.
- A minimum of 3-5 years of experience in leading a team.
