This position requires a unique blend of skills, including counselling, coordination, and administrative duties. You will work closely with residents, families, government agencies, and community partners to ensure seamless care delivery.

Key Responsibilities:

  • Counselling for new referrals, assessing financial, health, and social circumstances.">
  • Coordinating resident admissions, orientations, and discharges.">
  • Managing onboarding processes, including appointment scheduling and subsidy applications.">
  • Performing frontline counter duties and handling walk-in enquiries.">
  • Building rapport and providing emotional support to residents.">
  • Assisting in developing individualised care plans.">
  • Monitoring National Means Testing for elderly residents.">
  • Liaising with families, government agencies, and community partners.">
  • Maintaining accurate documentation and reports.">
  • Contributing to programme planning and evaluation.">

Requirements:

  • A minimum Diploma in Social Work, Healthcare, or a related discipline.">
  • 1-2 years of experience in the Social Service Sector or a willingness to learn.">
