Job Title: Case Management Officer

Alexandra Health Pte. Ltd. is seeking a skilled Case Management Officer to join our team. As a Case Management Officer, you will play a vital role in providing assessment and intervention in management for patients' care.

Key Responsibilities:
  • Conduct biopsychosocial and financial assessments of patients/residents and recommend intervention plans to the multi-disciplinary team.
  • Participate and present cases in multi-disciplinary team/multi-agency case discussions to formulate common care goals.
  • Coordinate with service providers to ensure patients and families receive timely care support after discharge.
  • Provide information and assist with applications for services/schemes and financial assistance.
  • Escalate and handover case management to MSW timely when identifying suicide/family violence/addiction/abuse cases.
  • Make home visits for care assessments when necessary.
  • Document patient information, interviews, telephone calls, payments, and field visits for records.
  • Provide education and information on patients' conditions as required.
Requirements:
  • Must have professional qualifications in nursing, medicine, or para-clinical disciplines.
  • Must have prior job experience in nursing, medicine, or para-clinical disciplines that commensurate with the job.
  • Must have at least 5 years of working experience in patient-related care.
Working Conditions:
  • Health risk of being exposed to infectious diseases.
  • May be exposed to patients and visitors with varied mental health states and may have personality issues.
