Key Responsibilities: Facilitates parents’ understanding of programme and triangulates information with recommendation(s) from stakeholders. Collates and consolidates all information about the child and family from relevant stakeholders to provide a profile of the child through all settings. Participates in case discussions by providing information about child and family strengths and needs and identifying possible care plans required for the child and/or family. Collaborates with all caregivers/educators of the child (including parents/caregivers, teachers, and stakeholders) to support child and family development (e.g., organizing and/or participating in case discussions with a multi-disciplinary team). Completes documentation (e.g., case notes) accurately and communicates information effectively to relevant professionals, performing any other administrative support duties related to maintaining and sustaining casework supporting systems. Undertakes, performs, or participates in any other executive work assigned by PCF Management. Documents and maintains case files. Identifies service gaps in response to client needs and makes recommendations to improve service provision of programs. Casework Evaluation: Undertakes and performs any other administrative support duties related to maintaining and sustaining casework supporting systems. Participates in intra-case conferences as part of a multidisciplinary team. Assists in the development of programs and conducts basic analysis of the data collected. Casework Intervention: Understands relevant agencies offering and taps on community resources to provide intervention support to clients. Provides necessary support for families by bringing together resources. Monitors progress and documents. Collaborates across disciplines and sectors to bring about change to benefit families. Professional Development: Develops professional and engaged relationships with clients and significant others and gathers relevant information from clients. Participates in professional sharing during monthly meetings. Engages in learning opportunities (e.g., participates in community of practice meetings to develop skills and capabilities). Job Holder Requirements: Relevant Diploma related to child development or customer service from an accredited institution. 3 years of prior working experience in a related field. Comfortable working independently as well as part of a team, being both client-centred and service-oriented. Works with ease in an interdisciplinary team, including nurses, therapists, and early childhood educators. Adaptable in a fast-paced environment, with good written and verbal communication skills. Possesses excellent communication skills, is patient and empathetic, and has the ability to handle stressful situations calmly. Organized, detail-oriented, and able to work independently or as part of a team. Desires to participate in shaping an early childhood social work intervention model.
