
Social Health Integration Manager

As a senior assistant manager, you will play a pivotal role in advancing social-health integration and enhancing care delivery in the community. Your primary objective will be to develop strategic portfolios that support population health management.
The position holder will report to the manager and be responsible for driving innovation and collaboration within assigned initiatives and partnerships.
Key Responsibilities:
- Develop comprehensive strategies and workplans to advance social-health integration and augment the organization's role in new health policies.
- Identify opportunities for collaboration with stakeholders and drive shared successes through effective communication and relationship-building.
- Oversee project management of assigned initiatives and collaborations, ensuring timely completion and successful outcomes.
- Stay up-to-date with developments in the health and social landscape, applying this knowledge to inform strategic decisions.
- Provide secretariat duties to facilitate progress and conversations in assigned portfolios.
Requirements:
- Bachelor's Degree in a relevant field.
- 4-6 years of relevant working experience, including 1-2 years in a leadership position.
- Prior experience in planning, partnerships, and portfolio/product management is highly advantageous.
- Excellent communication, interpersonal, and organizational skills are essential for success in this role.
