As a member of our team, you will play a vital role in delivering exceptional care coordination services to individuals with unique needs. Your expertise will ensure that every client receives tailored support, empowering them to make informed decisions about their care.

Key Responsibilities:
  • Closely collaborate with internal stakeholders to facilitate seamless service delivery
  • Manage inquiries on our health services and coordinate requested services
  • Evaluate customer needs using established screening tools and recommend suitable service packages
  • Cultivate strong relationships with external partners to enhance service referrals and follow-up
  • Conduct regular home visits and telephone follow-ups to engage with clients and assess their progress
  • Provide education and guidance to caregivers on effective care strategies and available resources
  • Work closely with the Operations Manager to ensure smooth client discharge from services
  • Participate in program development and evaluation to continually improve our service offerings
