Job Role Overview:
The Case Worker is a pivotal role within our organisation, serving as the primary point of contact for applicants. This position entails processing new applications and conducting thorough financial assessments to determine the appropriate level of support.
Liaising with social workers and hospital coordinators, the Case Worker identifies key issues, refers patients to relevant services, and maintains accurate records of patient information.
Main responsibilities include effective debt management, contributing to organisational performance metrics, and providing assistance with tele-consultations for newly diagnosed cases.
To excel in this role, candidates must possess a degree in Psychology or a related field, demonstrate dynamic self-motivation, and have experience in social service or healthcare.
Key Responsibilities:
- Process new applications and conduct financial means tests
- Liaise with social workers and hospital coordinators
- Refer patients to internal and external services
- Document patient information in case-files
- Manage debt effectively
- Contribute to departmental KPIs
- Assist with tele-consultations
Requirements:
