As a Senior Resident Care Coordinator, you will play a vital role in ensuring the highest level of care for residents. Your responsibility will be to develop and implement personalized care plans that cater to their diverse needs and preferences.
Key Responsibilities:- Develop comprehensive care plans that address individual resident needs, taking into account their medical history, personal preferences, and lifestyle choices.
- Regularly review and record residents' progress to ensure optimal care outcomes.
- Collaborate with healthcare professionals and other stakeholders to design and organize programs and activities that meet residents' diverse needs and preferences.
- Prepare accurate and timely reports and records to monitor and evaluate resident care.
- Must have a degree in a related field (e.g., nursing, gerontology).
- Have at least 2 years of experience working in a healthcare setting.
- Proven ability to communicate effectively with residents, family members, and healthcare professionals.
- Strong organizational and time management skills.
