Job Title: Holistic Health and Wellness Specialist

We are seeking a skilled and compassionate professional to deliver exceptional patient care in the healthcare sector.

  • The ideal candidate will have relevant working experience in a healthcare setting, preferably in a similar capacity. A diploma in Health Sciences (Health Management) or other relevant courses such as sports sciences, therapy aide, or community care associate would be highly advantageous.

Key Responsibilities:

  • Develop, implement, and promote wellness programs that cater to the diverse needs of patients.
  • Oversee patient screenings, conduct physical assessments, analyze data, and provide recommendations for follow-up care.
  • Work collaboratively with the Clinical Services Team to identify and address barriers to participation, ensuring patients receive adequate support in their wellness journey.
  • Conduct routine reviews to track patient adherence and progress in the exercise program, ensuring continuity of care plans.
  • Monitor and manage patient attendance, ensuring accurate and timely documentation to support consistent program participation.

Requirements:

  • Prefarable Diploma in Health Sciences (Health Management) or other relevant courses (e.g., sports sciences, therapy aide, community care associate).
  • Relevant working experience in a healthcare setting is advantageous.
