Job Title: Client Support Specialist
We are seeking a highly skilled and dedicated individual to join our team as a Client Support Specialist. The successful candidate will provide exceptional support and assistance to Senior Medical Social Workers in identifying and implementing effective interventions to reduce the burden of clients and families from incurred social and healthcare costs.
Key Responsibilities:
- Liaise with clients and families to follow up on pending assistance, including Medifund and Household Means Testing (Subvention) funding schemes.
- Provide expert advice to clients and their families on eligible resources to tap into.
- Support and adhere to care plans developed by S/MSW (Centre Manager) and other healthcare colleagues.
- Monitor clients' social wellness and participation rate in social activities during their stay.
- Assist in follow-up and liaison with community services in the discharge care plan of clients, if any.
- Offer financial counselling to all clients during assessment.
- Closely monitor KPIs for referral intake, ensuring timely referrals within 10 days.
- Provide administrative support by maintaining records, filing, and updating progress reports.
Requirements:
- Diploma in Social Work or relevant field.
- Fresh graduates welcome, with a minimum of 1 year of relevant healthcare industry experience.
- Pleasant and mature disposition, with empathy and patience when interacting with clients and colleagues.
- Excellent communication and interpersonal skills.
- Linguistic capability in English, Mandarin, and local dialects (advantageous).
- Strong organisational abilities.
