Job Summary

We are seeking a dedicated and compassionate professional to fill the role of Social Support Coordinator.

About the Role

The successful candidate will provide supportive services to residents in the aftercare phase of our program, working closely with our team to deliver an integrated service as part of a community-based initiative.

  • Main Responsibilities:
  • Conducting pre-emplacement interviews with residents to understand their needs, set goals, and develop case plans to address concerns.
  • Preparing and maintaining individualized service plans, progress reports from incare to aftercare phases.
  • Referencing complaints, issues, or requests from residents to social workers/ managers when necessary.
  • Motivating residents through daily interaction and ensuring their needs are well taken care of.
  • Monitoring workplace safety and health standards.
  • Providing administrative support for planning and conducting programs and activities for residents and staff in both incare and aftercare phases.
  • Supporting administration, operational, and engagement of volunteers.
Required Skills and Qualifications

To be considered for this role, you will need:

  • A minimum diploma in a related field of study, preferably in social work, counseling, or psychology.
  • Proficiency in Microsoft Office Suite.
  • Excellent project and event management skills.
  • Good verbal and writing communication skills; bilingualism is an advantage.
  • Relational skills with an ability to adopt an empathetic approach with people from complex backgrounds.
  • Good leadership skills for directing volunteers and overseeing projects.
